The card is evidence. The entitlement is the thing. People spend weeks chasing the card when the actual problem is that the entitlement behind it has not been established — and those need completely different fixes.
Card and entitlement are different problems
If you have an entitlement and no card, that is administrative — a process to complete, evidence to supply, a queue.
If you have no entitlement, no amount of chasing the card will produce one. The question moves to why: your route, your status, your registration.

It is regional, and that is not a detail
Spain's public healthcare is administered by the autonomous communities. Each has its own service, its own name for it, and its own way of handling a new arrival.
A guide that tells you which office to visit and what to bring is describing one community, usually without saying which.
The useful move is to find your own community's health service and work from its instructions rather than from a general article — including this one.
The out-of-date advice problem
Spain's framework for who gets public healthcare was restructured. The older model sorted people into insured persons and their dependants, and much English-language writing still explains your position that way — typically "you're covered as a beneficiary because your spouse pays Social Security".
Those provisions were repealed and access was reorganised around universal access, with separate provision for non-residents.
What we will say is: check the date on anything explaining your public position, and treat the beneficiary framing as a sign the text predates the change.

Emergency care does not ask for the card first.
