If you are coming to Spain to be employed, the health-cover question has a different shape from the one asked by someone living here without working. Employment brings you into the Spanish system through the act of working, and the private-policy requirement people search for belongs mostly to routes where that does not happen.
Why the question is shaped wrongly
Because "Spanish visa health insurance" is treated as one topic. It is not one topic — it is several conditions in several instruments applying to several different kinds of person.
The clearest contrast is the non-lucrative route, which exists precisely for people not working in Spain and which does carry an explicit insurance condition: five words in the regulation, have a sickness insurance policy. An employment route is the opposite case. What Spanish law actually requires separates them.
