People arrive at this question holding a document and expecting it to be an answer. An S1 is a real thing and it matters — but it changes the shape of the question rather than closing it.
What an S1 is
It is a document issued by one state to say that another state should provide healthcare at the first state's expense. It belongs to the coordination of social security between states, not to Spanish immigration law, and not to the insurance market.
That is why it feels like it should settle everything — and why it often does not on its own.
Why it does not simply end the question
Entitlement still has to be established in Spain. A document in your hand is the beginning of a process here, not a substitute for it. Until that registration has actually happened, you may be in a gap: holding a document that says you should be covered, while not yet being covered.

The honest formulation is "may". Not everyone who expects an S1 gets one, and not everyone who gets one is covered from the day they land.
The out-of-date advice problem
A great deal of writing about Spanish public healthcare describes a system that has been changed. The older framework divided people into insured persons and their beneficiaries, and advice built on it explained how to be covered as somebody's dependant.
Those articles of the old regulation have been repealed, and Spain's access framework was reorganised around universal access. If you are reading something that explains your position in terms of being a beneficiary of a contributor, check its date — it may be describing a structure that no longer governs anything. Healthcare in Spain is the current picture; public or private is the comparison.
So which do you need?
If your public entitlement in Spain is established and in force, that is cover. The regulation asks for cover covering all risks in Spain; it does not insist that the cover be private.
If it is not yet established, you may still need private cover — for the registration, for the gap, or for both. That is a sequencing problem rather than a legal one, and it is the thing most people underestimate: the application often comes before the entitlement.
