This page is about two decisions, not about pregnancy care. Which system you use, and the insurance timing that surprises people — because that one has to be settled much earlier than feels necessary.
The timing trap
Maternity cover in a private policy very commonly carries a waiting period, and it tends to be one of the longer ones. We state no duration and describe no policy — how waiting periods work explains the mechanism and why we will not quote one.
The consequence is simple and badly understood: if you intend to use private cover for maternity, the policy generally has to have been in force well before you are pregnant. Buying when you find out is usually too late for that benefit.
If you are moving to Spain and planning a family, that moves the insurance decision much earlier in your planning than most people put it — into the same conversation as the visa, not after it.
Public or private
Both are used, and the choice is not obviously one-sided.
Public maternity care in Spain is well regarded. Private cover buys the things private cover generally buys — choice of practitioner, continuity, often language. Public or private healthcare is the broader comparison and English-speaking doctors is the language question, which many people weigh more heavily here than anywhere else.
Plenty of people use the public system and hold private cover alongside, which is the normal Spanish pattern rather than a hedge.
