The structural difference between Spain's two systems shows up here more clearly than anywhere else, and it is one of the main practical reasons residents hold private cover alongside public access.
The public route
Your assigned primary-care doctor is the gateway. They assess, and where appropriate they refer onward within the public system.
That is a clinical decision, not a form. The commonest frustration — asking for a referral and not getting one — is usually a disagreement about clinical need rather than an administrative obstacle, and it is worth understanding it that way before escalating.

The private route
Many private policies allow more direct access to specialists, and that — more than the quality of emergency care — is what people are buying.

And direct access is not unlimited access. Authorisation requirements, networks and sub-limits are all common, and all in the document rather than on the sales page.
Using both, which is normal
Plenty of residents hold public access and private cover and use each for what it is good at. That is the ordinary Spanish pattern rather than a hedge: the public system for continuity and serious events, private cover for speed and choice on the things that are not urgent but are not nothing.
It also means a specialist seen privately may recommend something that happens publicly, or the reverse. Keeping both sets of records yourself makes that far less painful.
