There is no list. Not because nobody has written one, but because Spain does not set requirements for "a Spanish visa" — it sets them per route, in different instruments, and they genuinely differ.
The single most useful thing on this page is a demonstration of that, using the one condition everybody asks about.
The same question, three answers
Non-lucrative route. The regulation's condition is five words in Spanish: Contar con un seguro de enfermedad — have a sickness insurance policy. No benefit list, no product specification.
EU registration. A different regulation asks an economically inactive citizen for insurance covering all risks in Spain — a scope requirement with a geography in it.
Self-employment route. We searched the governing articles of the general regulation and found no health-insurance condition in them at all.
Three routes. One question. Three different answers, and one of them is "nothing". Anyone handing you a universal Spanish visa requirements list has not looked. What Spanish law actually requires sets the regimes side by side.
Why the folklore exists
Because one route's conditions get repeated as everyone's. The strictest published standard — comprehensive cover with no copayments, no reimbursement model and no waiting periods — is real and official, and it was written for study stays.
It has been repeated so widely that people now apply it to routes where the governing regulation does not mention those conditions at all. The words for copayment and waiting period appear zero times in both the non-lucrative and the EU regulations.

Identify your route first. Everything else follows from it, and nothing before it is reliable. Which route lets you live in Spain.
Then read the instrument for that route, not a summary of "Spanish visa requirements".
