The Medical Certificate: Not the Insurance One

Two different documents get called "the medical" and they are unrelated.
The medical certificate is a statement from a doctor about your health. The insurance certificate is a statement from an insurer about your cover.
Confusing them means arriving with the wrong paper, which is a wasted appointment. The insurance certificate is the other one.
What it is for
Broadly, a statement that you do not present a public-health concern of the kind the international health framework is concerned with. It is a screening document, not a full medical report, and it is not an assessment of whether you are healthy enough to live somewhere.
Required phrasing is set by whoever receives the document, and it is one of the places where an almost-right document is rejected.
Why we will not give you a template
Because a wrong template is worse than none. Pages circulate with model wording copied from one post's requirements years ago, and a doctor who signs that text may produce something your post will not accept.
Ask the body that will receive it what it requires, then take that to the doctor. One email ahead of the appointment prevents the entire problem.
The practical traps
Who may issue it. Some posts specify the kind of practitioner, or require particular registration. We state no rule.
Language. It may need to be in Spanish, or bilingual, or translated — and a sworn translation is a different thing from a translation. The documents and their traps.
Validity. It is usually only accepted if recent, which makes it one of the documents to obtain last, after the slow ones are finished. How the application works is the sequencing.
Cost and availability. Your usual doctor may not provide this, and finding one who will can itself take time.
What it does not do
It does not establish that you have health cover. That is the insurance condition, which is a different requirement in a different place — and on some routes does not exist at all. What Spanish law actually requires.
And it is not a health assessment you can fail by being unwell. It is a screening document with a narrow purpose.

Confirm whether your route and post require it at all. ⛔ We do not tell you that, because it varies and we have not verified it. Asking first is free; a certificate you did not need is not.
